The Site is provided for general informational purposes and to allow visitors to learn about our services and request appointments. The Site does not offer products or services for sale online, and submitting an appointment request through the Site does not guarantee that an appointment will be scheduled or confirmed. We will contact you directly (by phone or email) to confirm any appointment.

The content on this Site — including text, graphics, images, and any other material — is for general informational purposes only and is not intended to be, and should not be construed as, medical advice, diagnosis, or treatment. Use of the Site, including submitting an appointment request, does not create a therapist-patient or provider-patient relationship. That relationship is established only once you have been evaluated in person (or via a scheduled telehealth visit, if applicable) by a licensed provider at our practice.

Submitting an appointment request through the Site is not a confirmed booking. All requests are subject to review, provider availability, and confirmation by our staff. We may decline to schedule an appointment for any reason, including if the services requested are not appropriate for your needs. Please do not include sensitive medical details in an appointment request form; standard, unencrypted email and web forms are not a secure or HIPAA-compliant method of transmitting protected health information, and any such details you choose to share are provided at your own risk.

You are solely responsible for your User Content. By submitting User Content, you represent that you have the right to submit it and to grant the license described above, and that the User Content does not infringe or violate the rights of any third party. We may, but are not obligated to, remove or decline to use any User Content at our discretion. If you submit a testimonial or review involving your own health condition or treatment, we will only use identifying details (such as your name or photo) with your specific, separate consent at the time of collection.

 

10. Copyright / DMCA Notice Procedure

 

The Company respects the intellectual property rights of others and expects users of the Site to do the same. THIS SECTION DOES NOT LIMIT LIABILITY FOR CLAIMS ARISING FROM IN-PERSON CLINICAL CARE, WHICH ARE GOVERNED SEPARATELY BY APPLICABLE PROFESSIONAL STANDARDS, INFORMED CONSENT DOCUMENTATION, AND LAW, RATHER THAN BY THIS WEBSITE AGREEMENT.

You agree to indemnify and hold harmless the Company, its owners, employees, and agents from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising out of your violation of these Terms or your misuse of the Site.
